❤️ Chronic Disease Management (CDM)
We work with clients under GP Management Plans (CDM plans) to manage long-term health conditions such as arthritis, COPD, cardiovascular disease. our goal is to improve physical function, reduce symptoms, and enhance day-to-day living through tailored exercise programs and education. Medicare rebates available for eligible clients with GP referrals.
📦 Home Care Package (HCP) Physiotherapy
We work with approved HCP providers to deliver physiotherapy services for older adults receiving government-funded home care. Our focus is on falls prevention, strength and balance training, pain relief, and functional independence. We tailor programs that align with your care goals and budget—helping you live safely and confidently at home. We can also assist case managers with clinical reports and recommendations for mobility aids and home modifications.

Nirmal specialises in aged care, chronic disease management, home-based therapy, and disability support through the NDIS. Known for his practical problem-solving and clear communication, he works closely with clients, families, care coordinators, and providers to deliver meaningful outcomes.
